Pre-chat form to collect visitor name and email

As a support agent, I want visitors to be required to enter their name and email before they can start a new chat conversation. Currently, anonymous visitors are given an auto-generated name, which makes it difficult to identify and communicate with them personally. A pre-chat form would solve this by collecting essential contact information upfront.
