Link Changelog to a post

It would great to link a post to a changelog. This way, anyone who sees the post can easily find the changelog describing the update. Ideally, the link could handle one-to-one (e.g. one post = one changelog) and one-to-many (e.g. multiple posts are mentioned in one changelog)
