[Discord Feedback] Allow attachments and custom fields in Discord feedback modal

I would like to allow submitters to attach images or videos directly within the Discord feedback modal.

This functionality is currently available through the feedback portal, and it would be a great addition to the Discord integration, especially since Discord has recently updated its components.

Furthermore, it would be beneficial if the Discord modal could support custom fields set on the backend of Featurebase for synchronization.
