Specific changelog entry link in Post comment

When I implement a requested feature, I publish a long changelog introducing the feature and how to use it.

I'd like to be able to get a direct link to this changelog entry to sent to my customer and add as a comment in the "Post".

It would be even greater if this last part could be done automatically by "linking" the changelog to a Post when I write it
